As for the covers, I have some say in what they go with, but only some. The PA cover was one that I didn't much care for at first, but has since grown on me. Most of the people who've commented on the cover specifically have said they found it very striking graphically. I think the biggest problem I have with the PA cover is that it misrepresents the book, though not the title, if that makes sense.
